How to Use the Risk Mapping Template

Step 1: Sketch Your Workplace Layout

  • Draw a simple floor plan of the area you are assessing (it doesn’t need to be to scale).
  • Include key features such as entrances, workstations, shelves, and pathways.

Step 2: Mark Hazardous Areas

  • Use crosses (X) to indicate places where slips, trips, or falls have occurred.
  • Use crosses (X) to indicate hazards, such as uneven flooring, leaks, loose cables, or poor lighting.

Step 3: Investigate & Record Causes

  • Talk to employees about any incidents or near-misses they have experienced.
  • Identify what caused the hazards (e.g., wet floors, damaged tiles, cluttered walkways).

Step 4: Implement & Monitor Control Measures

  • Record the actions needed to fix each hazard (e.g., replacing a tile, fixing a leak, adding warning signs).
  • Discuss findings with management and assign responsibilities.
  • Regularly review and update the risk map to maintain a safe working environment.
